At zmp neurophysio we will collect personal information about you so that we can support your treatment process or gather relevant information to handle your enquiries. This information is collected directly from you and may also be collected from your next of kin or carer with your consent.

- Reason for requesting Physiotherapy input
We will also document your assessments and treatments on electronic notes as you continue to be seen by us.
We may collect this information in various ways depending on the situation:
- Over the telephone – collecting personal information, responding to a general enquiry from a client and carrying out a triage to see if they are suitable for our service
- Via email – responding to a client’s enquiry
- Via whats app – responding to a client’s enquiry
- During face to face visits
How we use your personal information and data sharing
We use the information that you give us to understand how your condition is affecting you, what your needs are, and to ensure that we can tailor our treatments to best suit your needs and goals. It also allows us to provide you with a safe and effective service. On occasion we may need to contact those involved in your care to ensure that the treatment we are providing protects your health. We will notify you if this necessary.
With your permission we may share your information with other health professionals who are involved in your care e.g. your GP or consultant. This information may be given in a form of a letter. If this is the case, the letter and the protection of its contents becomes your responsibility. There may be times when we may share your information with health or social care professionals when we are concerned that you are at risk to yourself or others and in this instance, we do not need your consent to share relevant data, however we would inform you of this decision.
To comply with lone working policy daily appointments with your name, address and contact number will be available to view from the physiotherapists next of kin and will only be accessed in case of emergency.
We will also collect personal information to obtain payment for our services. Copies of the invoices are also kept electronically for accounts and tax purposes.
We may also use your details as part of our audits to help us to improve our treatments and services to individuals like yourselves.
We may use your personal information to contact you in the future about promotions about our services we think you may be interested in.
Security of information
The protection of your personal information is vitally important to us. We will strive to protect your Personal Information in all means reasonably required by us to do so. However, as no form of data transmission is 100% reliable, we cannot guarantee its absolute security. We will however always act in accordance with the relevant UK and EU legislation.
We store personal information on secure servers that are managed by us, and occasionally hard copy files that are kept in a secure location. We have a legal obligation to retain our client records for a period of eight years after the last point of contact. This is in accordance with CSP and HCPC professional guidelines.
